Forensic Analysis of A CNC Lathe Window Guard Failure

An analysis of an industrial lathe accident is presented in this paper. During operation, the rotating chuck lost its grip upon cylindrical workpiece. The detached workpiece impacted inside cabinet, rebounded, struck composite viewing window, and penetrated it. While observing machining process through machinist was chest by He fell backward, suffering a severe traumatic brain injury when back his head concrete floor. forensic incorporated event reconstruction to include estimate ejection angle, velocity, kinetic energy workpiece, impact capacity failure window frame.
